This post will be based at the Royal Gwent Hospital, working Monday- Friday. The post holder will contribute to the provision of CAATT by providing high quality, effective support to people who have presented to the emergency care department/acute medical units. This activity supports the ABUHB Clinical Futures work aimed at preventing unnecessary admissions to hospital, reducing length of hospital stay, providing care closer to home and establishing home-based support where appropriate by liaising with primary, community and third sector organisations Main duties of the job This role is recognised as a key part of the multi professional service delivery and we are looking for skilled, organised and enthusiastic individuals to join us. Patient management will include responsibility for the delivery of therapeutic interventions as delegated by the registered therapists along with close liaison with patients, family, members of the MDT and external agencies. Post holders will need to demonstrate good time management, prioritisation skills and flexibility in their approach as well as the ability to communicate effectively both verbally and in writing. Successful candidates will work alongside registered Occupational Therapists, Physiotherapists and Dietitians and will manage a delegated caseload of patients. The role will include supporting the assessment and intervention plans of people with a range of functional difficulties within the bounds of the practitioner role, including those presenting with multiple co-morbidities, frailty and cognitive deficits The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
